The following format of sanction letter shall be as follows:-

(a) For the purpose of clause (b) of sub-section (1) of section 15 as per Schedule-13 for,

(b) as per Schedule-14 for the purpose of clause (b) of sub-section (1) of section 16 of the Act,

(c) After the death of the donor who gives the permission in accordance with clause (b), the permission given by his next of kin to remove the kidney from his body as per Schedule-15,

(d) for the purpose of sub-section (1) of section 19 as per Schedule-16.
